So, when dealing with mistakes in business, focus on the cause rather than the mistake.
That is the only way to reduce or eliminate similar problems in the future.

But that's not what most managers do. For many it's much more important to fix blame than it is to fix the problem.

They may say it but in the end, far too many simply want to find someone to blame.
